| 摘要:公立医院改革的重点是控制医疗费用和规范诊疗行为，让患者真正得到实惠。公立医院改革目前面临的主要问题是现行治理结构难以实现有效管理，医疗费用支付方式难以有效控制医疗费用，医保资金在控制医疗费用、规范医疗行为上的作用没有得到很好的发挥，高值耗材和药品费用在医疗总费用中仍占较大比重。本文认为公立医院改革应从四方面进行实质性操作，即改革治理结构，实行管办分开；改革支付方式，落实临床路径；发挥保障基金经济杠杆作用，实施分级医疗；实行药品及耗材统一招标采购，落实医药分开。 |

| The operation suggestion on public hospital reform |

| Abstract:The priority of public hospital reform in China aims to focus on control of medical expenditure and the adjustment of health care provider’s behavior In order to improve benefit to the patients. It is faced with four problems in line with the reform. First, the governance structure does not meet the needs of effective management. Second, the payment mode and medical insurance does not take effect on controlling medical expenditure and adjusting the health care provider’s behavior. Finally, the cost of high vale consumables and drug still accounts for relatively high proportion of the medical expenditure.The paper suggests to change the governance structure, separate management and regulation; change the payment model, adopt the clinical pathway; promote the regulatory role of medical insurance fund, implement patient care classification; drug and consumables procurement be carried out through the system of government purchases and open bidding, separate prescribing and dispensing. |
