| 摘要:总额预算制度日益成为大陆控制医疗保险基金收支平衡的重要制度安排，但少有文献介绍海外总额预算制度。文章分析评价了台湾全民健康保险总额预算制度的实施背景、预算确定方式和监管制度。同时，对比了台湾和大陆政治、经济、社会及医疗卫生体制的异同，认为大陆在当前制度环境下，不宜全面推广总额预算制度，而应在建立健全总额预算的制度与组织架构以及总结应对医疗机构道德风险的有效措施两方面加强试点，为未来实施总额预算制度奠定基础。 |

| Abstract:The global budget system (GBS) is becoming an important part of controlling medical insurance expenditure on the Chinese mainland. However, there is little literature introducing the overseas GBS. This paper discusses the background, approaches and regulatory system of the GBS of the national health insurance system in Taiwan. This paper compares similarities and differences in terms of the political, economic, social and healthcare system of the Chinese mainland and Taiwan. We suggest that China should not implement GBS as fast as it could in the current institutional environment. We should experiment on two aspects to prepare for the implementation of GBS in the future: establishing a sound system and organizational structure of the GBS and summarizing effective measures to deal with moral hazard of the medical institutions. |
