| 摘要:目的：应用扎根理论方法探讨贵州省医疗联合体建设和发展的现存问题并提出对策，为医联体改革提供参考依据。方法：2017年7—10月对贵州省9个市（州）39家医疗机构的76名医院管理者、临床医生进行半结构化访谈，采用三级编码提取、归纳医联体建设和发展的现存问题及对策。结果：经过三级编码，梳理出138个概念，21个范畴、6个主范畴，最后形成1条故事线：医联体建设和发展受多种因素的制约，政策支持是外部条件，加强内部组织与管理及学科间联系是内部要素，保障核心医院合理利益诉求是关键环节。结论：医联体应以资源整合和利益共享为基础，学科建设为导向，人才队伍建设为核心，成员单位共同发展为目标。 |

| Analysis of existing challenges and countermeasures in the construction and development of regional medical union in Guizhou province: A qualitative research based on Grounded Theory |

| Abstract:Objective:This paper aims to analyze the challenges and countermeasures in the construction and development of regional medical union (RMU) in Guizhou province based on grounded theory, in order to provide practical references for further reform of RMU.Methods:From July to October in 2017, a total of 76 persons including hospital administrators and clinicians selected expediently in 39 medical institutions at 9 districts in Guizhou province were interviewed using semi-structured questionnaire about the construction and development of RMU. In this paper, the existing challenges and countermeasures are extracted and summarized through three-grade coding. Results:After three-grade coding is conducted, 138 concepts, 21 sub-categories and 6 main categories are revealed. The results show that, the construction and development of RMU is constrained by many factors, of which policy support is an external condition, strengthening internal organization and management and interdisciplinary linkage sare internal factors, and ensuring the legitimate interests of core hospitals is the key link.Conclusions:RMU should be based on resource integration and benefits sharing, orient to discipline construction, focus on personnel training, with the goal of promoting the development of all member hospitals. |
