家庭医生签约服务：治理困境与机制构建

摘要:政府主导和市场调节的双重制度环境下，家庭医生签约服务通过政策引导和自主签约方式提高医疗服务水平。基于政府主导的签约服务限定服务内容、规制服务定价、维护服务秩序，与基于市场调节的自主签约带来的专属个性化服务相得益彰，产生了具有特色的医疗服务模式，却也面临着医患信任和风险防控的治理困境。构建以互动、整合和信任为特色的网络治理结构，理顺政府与市场的角色关系，对市场和社会资源进行跨界、跨区域整合，推进政府向社会赋权，发挥社会组织的有效作用，不失为推进家庭医生签约服务发展的有效路径。

Family doctor contracting services: Governance dilemma and mechanism construction

Abstract:Under the dual system of government-led and market-adjusted, family doctor contracting services improve the level of medical services quality through policy guidance and individual and independent contracting. Based on the government-led contracting to provide certain limits of service content, regulate service pricing, maintain service order, and the exclusive personalized service brought about by independent regulation based on market demands, the system has produced a distinctive medical service model, but also faces a big governance dilemma on the trust between doctors and patients, as well as of the risk mitigation. In order to empower the society and play an effective role in social organization, it is of great impetus to build a network governance structure which is characterized by interaction, and integration and trust; to rationalize the role relationship between the government and the market, cross-border and cross-regional market and social resources integration; and to promote the government's society empowerment. Therefore, this policy was found to be an effective way of promoting the development of family doctor relationship under the contracting services.
