| 摘要:本文基于波特钻石模型，从生产要素、需求条件、相关及支持产业、战略结构和同业竞争、政府、机会六个要素分析比较了县域医共体背景下浙江省东阳市、余姚市、桐乡市、德清县县级妇幼保健机构的发展现状。结果发现，县级妇幼保健机构的生产要素主要取决于发展模式和资源整合，县域医共体主要影响其机构协同和需求条件，政府在其中发挥着主导作用。因此，建议保健与临床相结合的县级妇幼保健机构中，大专科小综合的应牵头相应规模的医共体，综合技术较弱的应通过参与医共体加强与综合医院技术协作；县级妇幼保健机构特别是保健模式应立足妇幼需求与保健特色，创新服务模式与内容；县域医共体建设中应建立妇幼健康指导团队确保县级妇幼保健机构公共卫生功能实现。 |

| Comparative analysis on the development of county-level maternal and child health institutions of Zhejiang Province in the context of county medical community |

| Abstract:This study comparatively analyzed the development of county-level MCH institutions of Dongyang, Tongxiang, Deqing, Yuyao counties of Zhejiang province in the context of county medical community based on the Michael Porter Diamond Model which include six elements: production factors, demand factors, relevant and supporting industries, enterprise strategy, structure and peer competition, government factors and opportunity. The analysis result showed that the production factors of county-level MCH institutions depend principally on their development model and county health resource integration. The influence of county medical community on county-level MCH institutions lies in the coordination of relevant institutions and demand factors. The government plays a leading role in the specific reform measure of resource integration and county medical community. Therefore, this study suggests that for the county-level MCH institutions that combine health prevention with clinical services, those with strong comprehensive medical technology should lead the corresponding scale of medical community, otherwise, they should strengthen the technical cooperation with general hospitals by participating in the medical community. County-level MCH institutions especially for those only provide preventive care services, should explore the health needs of MCH people, innovate service modes and provide more preventive services. The MCH guidance team should be established to ensure the realization of public health function of MCH institutions. |
