| 摘要:本文基于非对称博弈理论，剖析我国医疗机构中医师与药师在用药管理环节中产生的博弈现象，分析该背景下药师执业的困境。研究发现，药师执业存在权力弱化、专业意见难以落实、审方环节流于形式、激励机制不成熟以及风险共担缺乏界限等问题，制约了合理用药治理体系效能的发挥。本文提出以下对策建议：一是通过立法明确药师执业权界，强化药师在处方审核中的实质性参与；二是优化前置审方与信息共享机制，构建医师药师同步决策模式；三是调整绩效激励与服务补偿制度，建立反映药学价值的薪酬和晋升体系；四是区分责任承担，建立权责对等框架；五是增设处方争议处理机制，确立专业协商与流程化处置规则。 |

| Dilemmas， causes and optimization pathways of pharmacists' practice from the perspective of asymmetric game theory |

| Abstract:Based on asymmetric game theory， this study analyzes the strategic interactions between traditional Chinese medicine practitioners and pharmacists in medication management within Chinese medical institutions， examining the challenges pharmacists face in this context. Research reveals that pharmacist practice faces challenges such as diminished authority， difficulties in implementing professional recommendations， a perfunctory prescription review process， immature incentive mechanisms， and unclear boundaries in risk-sharing arrangements. These issues constrain the effectiveness of the rational drug use governance system.The study proposes the following countermeasures： First， legislate to define pharmacists' professional scope and strengthen their substantive involvement in prescription review. Second， optimize pre-review and information-sharing mechanisms to establish a synchronized decision-making model between physicians and pharmacists. Third， adjust performance incentives and service compensation systems to establish remuneration and promotion frameworks reflecting pharmaceutical value. Fourth， differentiate responsibility allocation to create a framework of equal rights and responsibilities. Fifth， establish prescription dispute resolution mechanisms with professional consultation and standardized procedural rules. |
